1. Table 10.2 (pg. 318) details Why Transformation Efforts Fail.  Describe a time or experience in our career that you have experienced a failed transformation utilizing the reasons detailed on Table 10.2.  Provide details to the examples you utilize.

Chapter 11:

1.  On page 359 of the text, Mitchell Marks and Robert Shaw (1995) argue that "productive failure" is valuable, if an organization has the capacity to add the learning from such experiences to its store of knowledge rather than to conduct a witch hunt to find who to blame.  Do you agree? If so, is this realistic and applicable in the real world?
