You work for a large healthcare system as a PMH NP.

In the mornings you work in the outpatient clinic.

You have 7 patients on your morning schedule – see below:

0800: Medication management for a 32 yo female with Schizophrenia, obesity, on oral contraceptives

0830: Established patient, hospital D/C follow-up for a 64 yo male with Major Depression with psychotic features, HTN, Type 2 DM, COPD

0900: Medication management for a 44 yo female with Schizoaffective D/O Bipolar type, migraine HAs

0930: Medication management for a 23 yo male with Bipolar I D/O with psychotic features

1000: New patient, hospital D/C follow-up for 25 yo male with Schizoaffective D/O, paranoid with AVH, substance use

1100: Medication management for a 37 yo female with Panic Disorder, endometriosis

1130: Medication management for a 16 yo male with ADHD, asthma

Reflect on the Cracking the Code presentation for E/M services from week 2. 

Consider the amount of time allotted for each visit and the anticipated elements of medical decision making that may be involved for each encounter.

Construct an initial discussion post that includes the following:

Assign an E/M CPT code for each patient visit.

Provide a justification for the code you assigned (e.g. based on time, based on anticipated medical complexity, etc.)

Indicate the anticipated reimbursement amount, based on the information provided in the week 2 Cracking the Code presentation.

Construct a discussion that includes the following:

Assignment of the Evaluation and Management (E&M) CPT Code for each of the patient encounters

Justification of the CPT Code you assigned for each patient encounter

Resources utilized for determining the E&M CPT codes; if an online resource include the link to the site.

Responses need to address all components of the question, demonstrate critical thinking and analysis and include peer-reviewed journal evidence to support the student’s position.
