For this assignment, you will select a Healthy People 2030 goal and consider the role of public health nurses in achieving this goal. Select a goal and a specific population that is impacted by this goal, and the health issue it targets. In a 1,250- to 1,500-word paper, provide an analysis of the population and discuss how the goal and related health issue affects them. Include the following:

 Describe the issue and the population that it affects. Why is this a vulnerable population? Include demographics of the population and why this issue is prevalent in this population.

Analyze the vitality and social determinants of health for the population. Consider indicators of social and economic conditions, health resources, environmental conditions related to health, social functioning, and attitude toward health care.

Discuss a proposed intervention and discuss how its benefits the target population. Consider any drawbacks or shortcomings of the intervention as well. Include in your discussion evidence that the intervention is in line with the core functions and ten essential public health services.

Performing a public health nursing assessment is a critical first step in determining health needs and appropriate interventions to meet them. For this assignment, you will perform a public health nursing assessment on a selected population using the Public Health Nursing Tool (PHNAT) included in Chapter 3 of the textbook. You are free to use your setting/population of interest discussed in Topic 3 DQ 1 or choose another population of interest. Consider the importance of preparing and providing quality care to the diverse groups within your target population.

Using the assessment framework provided by the PHNAT, develop a 1,500- to 1,750-word report summarizing your assessment findings and identifying the public health needs for your selected population. Your summary should address the following: 

Define your target population.

Describe data collection method and sources.

Analyze and interpret data regarding diversity of target population and correlation of health issues. Include socioeconomic, behavioral, biological, and environmental factors that influence a population's health and readiness for health promotion and disease prevention interventions.

Discuss cultural issues of diversity that may impact the community health.

Identify a specific community health need. Use 3-5 key data points from the assessment to support your rationale.

Recommend a specific intervention to address the identified community health need.

Defend why the 3-5 data points highlight the need for this intervention.

Describe how community stakeholders would be identified and engaged in the assessment process.

Propose how the assessment findings' documentation regarding the health issue could be disseminated to the community and its stakeholders.

Develop two specific improvement strategies based on a Healthy People 2030 initiative that could be used to create an initial improvement plan. Present the strategies in the form of two S.M.A.R.T. goals as a measurement.
