Overview

A prescription is an order from a physician to a pharmacist that indicates the medication that a patient is to take. If the prescription is poorly written, this can lead to medication error. Here are some statistics:

 

Medication errors occur in approximately 1 in every 5 doses given in hospitals.

One error occurs per patient per day.

Approximately 1.3 million injuries and 7,000 deaths occur each year in the U.S. from medication-related errors.

Drug-related morbidity and mortality are estimated to cost $177 billion in the U.S.

These are estimates from various sources and studies and provide shocking evidence about prescriptions. It is no wonder why malpractice insurance is so expensive?

CF is a 48-year-old man who presents for evaluation of heartburn. He denies current tobacco use but has a history of one ppd for 15 years. He consumes a glass of wine nightly, more on the weekends. He has a sedentary job.

He reports a burning feeling in his chest after eating. It is worse when he eats spicy foods or tomato sauce. He is sometimes awakened at night with these symptoms. He has tried over-the-counter antacids and histamine H2 receptor antagonists (H2RAs) with partial relief. He is on no regular medications. His examination today is normal. An upper gastrointestinal (GI) X-ray series reveals gastroesophageal reflux.

What lifestyle modifications do you recommend for CF?

RR is 48-year-old Hispanic woman who comes to see you at the V. A. homeless clinic. The major complaint is a six-day history of worsening cough, productive of green sputum. There have been fever and chills with complaints of pain in the right mid-back with deep breathing or coughing. Further history reveals a history of tobacco use of one pack of cigarettes a day for 30 years. There is an incontainent health history as a result of Post Traumatic Stress Disorder (PTSD) and homelessness. On examination, the skin is warm and clammy, pale; RR appears to not feel well; the temperature is 100.5° F; respiratory rate is 24 breaths per minute; pulse is 98 beats per minute; blood pressure is 120/75 mm Hg; and saturation of oxygen is 96 percent on room air by pulse oximetry.

Auscultation of the lungs reveals rales in the right lower-posterior lung field. The remainder of the examination is within normal limits.
