Using the "Public Health Nursing Practicum Documentation" form, write objectives of what you will accomplish in this topic and how you will evaluate your accomplishment. Refer to topic materials to be sure that your objectives are written using Bloom’s taxonomy.

You must align your practicum goals to Public Health Nursing Standards every 2 weeks. If your hours are completed earlier, in the remaining weeks where you are not completing hours, you can submit the documentation form with a statement saying that goals have been met and hours have been completed as of the date for the topic indicated.

Be sure to read the instructions (in blue font at the top of the columns on the first page) on how to use the form before you begin. The "Public Health Nursing Practicum Documentation" form uses the nursing process and follows the format of a nursing care plan, so this should be familiar to you.

The written objectives, resources, and criteria for evaluation columns (first three columns) must be completed prior to the practicum experience for the standards in Topics 11 and 12 (Public Health Nursing Standard 4 – Planning; Standard 6 – Evaluation; Standard 9 – Evidence-Based Practice and Research). Complete the fourth column (evaluation) at the end of each topic's practicum experience as you evaluate how you met the objectives you wrote.
