Use medication reconciliation as a significant practice gap and search for a total of 4 qualitative (or mix method) studies that address the practice gap. Use the Johns Hopkins Research Evidence Appraisal Tool, Section II Qualitative Appraisal to appraise each of the qualitative studies you have selected for your practice gap.
Once the appraisal tool is complete, write a 1,200- to 1,500-word summary paper that includes a title page and references page. Do the following:
· Describe the background on the defined practice gap.
· Discuss why an evidence-based approach is needed to address the practice gap.
· Provide a summary of each qualitative article and why you selected these articles.
· Provide a detailed summary of the appraisal of each article.
· Discuss how the evidence-based solution identified in the research supports your practice gap.
· Describe the data collected in each article and how the data was collected.
· From your assessment, describe the statistical method the researcher used to analyze the data.
· Describe if the statistical analysis used by the researcher was the best approach for this dataset.
· Describe the conclusions the researcher arrived at in each article.
· Based on your assessment of each article, define which articles will be included in your EBP change and which articles would be excluded.
· Provide a rationale for each.
· Provide a summary of what was presented in this paper.
