Introduction. Research on human factors provided a number of insights on why people do not speak up. Some of the common threats for speaking include:
A culture that discourages speaking up
Power gradient, as illustrated in the stories we heard from Kathleen Bartholomew from the last module. Some people are made to feel that they are not important
Lack of psychological safety, when people do not feel safe to speak up
Psychological safety is a very useful concept for safety improvement, as it uncovers an otherwise hidden variable that we need to improve up on. Amy Edmondson developed this concept, and explained it in a Ted Talk video below. She described anxiety zone as a dangerous place, in which people are afraid to speak up. She also described ways to build psychology safety: to be open for learning, to admit fallible, and to show curiosity. These will create an environment to draw people in, to solicit other people’s input. When watching video, think about your own workplace and whether you feel safe to speak up.
During the COVID-19 crisis, psychological safety is ever more critical to patient safety as healthcare professionals are facing more challenges. These include anxiety and fear over infection and spreading to their families. Policies and procedures are in constant adjustment resulting in frustration. They may face moral dilemmas as healthcare resource limitations force uncomfortable choices. A safe environment is critical for healthcare professionals to safely express their concerns, and to find support among each other.
