Anemia Case Study

Complete the case study by answering the questions associated with the scenario as if you are presenting to your preceptor.

Scenario

A 45-year-old perimenopausal Filipino female presents to the office with complaints of excessive fatigue and shortness of breath after activity, which is abnormal for her. The patient appears unusually tired and slightly pale. Additional history and examination rules out heart failure, acute illness, and kidney disease. She reports excessively heavy menstrual periods for the past six months. The CBC results indicate hemoglobin is 8.0 g/dL, which is a new finding, and the hematocrit is 24%. Previous hemoglobin levels have been 11 to 13g/dL. The patient’s vital signs are temperature 98.7°F, heart rate 92 bpm, respirations 28 breaths per minute, and blood pressure 138/72. As the practitioner, you are concerned about her new anemia and now need to diagnose and develop a treatment plan for this patient.

Case Questions

There are many types of anemia you may encounter in the primary care setting, such as anemia of chronic disease, iron deficiency, B-12/pernicious and folate deficiency, and thalassemias. Explain each type of anemia. What laboratory studies would you order, and how might they differ based on the type of anemia?
