Discharge Plan. Consider: A client, diagnosed with schizophrenia, is the subject of a discharge planning conference. The major focus of the discharge plan is concerned with maintaining independence in daily living and employment. From your knowledge of schizophrenia, list two specific issues that should be considered in the discharge planning for the client’s return to employment and community living.

a. Describe each of the two issues.

b. Provide a thoughtful intervention for each of the issues. Briefly identify who would be included in the plan, what is to be done, and identify at least one barrier to success.

c. Support your discussion with two literature resources in addition to the Text. One of the resources must be a scholarly journal; the other may be a professional website, i.e., American Psychological Association. Remember to use APA style to format your citation(s).

d. Respond to at least two of your classmates’ postings.
