Week 5 Systems Change Project

PURPOSE

The purpose of this paper is to provide the student an opportunity to design a system change project for quality improvement ofa patient safety issue. The focus of the project how the student would manage change using a change model, determine and evaluate outcomes, and sustain the change.

COURSE OUTCOMES

This assignment provides documentation of student ability to meet the following course outcomes.

CO2:Distinguish the impact of complexity, chaos, change, and innovation theory on the future of advanced nursing practice (PO #6).

CO8: Analyze mandatory reporting threats to the future of healthcare and patient safety improvements (PO #6).

CO9:Examine selected quality improvement and professional practice models that serve to support patient safety and advanced nursing practice (PO #6).

Federal and other open access sources are not included in the search functions and must be searched individually. 1.            The Systems Change for Quality Improvement of Patient Safety project is worth 250 points and will be graded on the quality of information, use of citations, use of Standard English grammar, sentence structure, and overall organization based on the required components as summarized in the directions and grading criteria and rubric.

2.            Create your report using Microsoft Word 2010(a part of Microsoft Office 2010), which is the required format for all Chamberlain College of Nursing documents. You can tell that the document is saved as an MS Word 2010 document because it will end in “.docx.”

4.            The length of the project report is to be no fewer than six and no greater than eight pages, excluding the title page and reference pages.

5.            APA format is required, with both a title page and reference page.

PREPARING THE PROJECT REPORT

The following are best practices for preparing this project report.

1.            When introducing the selected advanced nursing practice patient safety concern, be sure to include brief but pertinent background information regarding the patient safety issue and briefly introduce the change model you will use for the project (i.e., what is the problem, how do you know, what intervention will be used,and what change model will guide the implementation of the intervention).

2.            When describing the selected systems change for quality improvement proposal, be sure to fully identify and address each step of your selected change model (see Lesson from W2).

3.            When presenting the selected systems change for quality improvement analysis, be sure to fully address expected outcomes and the role of the DNP-prepared nursing leader in outcomes evaluation and sustainability of the proposed change.
