This activity is tied to:
Course objective 6:
- Course Objective 1: Apply nursing concepts and theories to the practice of health assessment.
- Course Objective 2: Incorporate a holistic and culturally competent approach to health assessment.
- Course Objective 3: Perform a comprehensive health history and physical assessment integrating both subjective and objective information.
- Course Objective 4: Analyze collected health assessment information to determine actual and potential risk factors and initiate an appropriate plan of care.
You will apply techniques of inspection and palpation as you assess your patient’s skin, hair, and nails, head, face, neck, nose, mouth, and throat. Auscultation techniques will be used in assessment of the neck. Examination of the eyes involves tests of visual acuity, visual fields, and extraocular muscle function. A penlight can be used to assist to with inspection of the external eye structures, anterior eyeball structures, throat, and mouth. An opthalmoscope is needed for examination of ocular fundus. An otoscope is needed to examine the external ear canal and tympanic membrane. If you do not have access to an opthalmoscope or otoscope, you can omit that portion of the physical examination on your patient.
- Course Objective 5: Communicate with interprofessional health care teams for further interpretation and recommendations to promote patients’ health.
You will apply techniques of inspection, palpation, auscultation, and percussion in this module. It is important to refer to your textbook for the proper sequence of examination techniques in each area that you are examining. For example, when you are assessing the abdomen auscultation should be performed before palpation and percussion because palpation and percussion can cause an increase in peristalsis, which would give false auscultation findings. If it has been awhile since you have auscultated lung or heart sounds, you may want to practice on friends or family members.
Review of auscultation of the heart and lung sounds are available through the following links:
- Normal heart sounds
- Abnormal heart sounds
- Bronchial and vesicular breath sounds
Developmental competence regarding differences in infants, children, adolescents, pregnant women, and aging adults is especially important in this module. Remember that the neurologic system is not completely developed in infants. They gain sensory and motor function as they grow and develop. For older adults, the process of aging causes decreases in sensory and motor function. Therefore, it is important to be able to understand the normal range of findings for the systems reviewed in this module, varies for each age group.
Objective 7:As a registered nurse, you already have experience with developing plans of care for patients. For the Module 7 assignment, each student will develop a PowerPoint presentation, utilizing a SOAP note format, to communicate the findings of the complete health history and physical examination on their selected patient.
Read Gagan's (2009) article and view the Soap Notes video to familiarize yourself with the components of a SOAP note before developing your presentation.Once you complete your PowerPoint presentation, you will submit it to Module 7: SOAP Note PowerPoint Presentation.
