The assignment begins with the Creedmoor Form:  This form is still used at Creedmoor Hospital.  I give this to you as a guide for asking questions.  Since this is such an extensive form, you need not ask all the questions.  There are areas where you should investigate with your person, like have you been abused, have you been the aggressor, have you used drugs and/or alcohol, etc.  Is there a family history of drugs/alcohol or mental illness.  If the person does not want to answer these questions, move on.  It will be difficult for most of you to ask these personal questions.  Give it your best attempt.

After your interview using the Creedmoor form as a guide, you will write up your findings.  It is not necessary to ask a client about their presenting problem.  These are volunteers, use the word issues.  We all have issues.

The template is in this section.  There are twelve areas that you will complete.

Strengths is the next item to be considered.  You should ask the client what are his/her strengths.  Some volunteers really don’t know how to answer this query.  “Well, I’m good at gardening” or “I’m fantastic when working on cars.”  Since you have completed your Professional Assessment, you know what strengths are. This is where you can try your skill at showing your client he/she has strengths based on the information you have secured. 

The last section of this evaluation is Your Impression.  In this section you should give an intervention or two capitalizing on the client’s strengths.   What options can you offer the client that will empower them.  Please don’t say “she needs therapy.” 

Above all, validate the client’s feelings.  Your client may be anxious and express that feeling.  Validate, explain that many people feel the same when they are starting something new, and try to move on.

Have fun.  Interviewing and assessing has always been my favorite part of Generalist Practice.

There are two examples listed in this Module, please read one or two.  They will be helpful to you.

This is the Template that you will use to interview a client (volunteer) and complete a psychosocial evaluation that will includes:

9) Coping

10) Spirituality

11) Strengths

12) Your Impression

 

After collecting all the pertinent information, you will write an essay describing this process and your reaction to this process.  Since social workers practice confidentiality, do not use the client’s full name or address in your essay.  *This assessment should be approximately 1,500 to 1,800 words in length and is worth 20% of your total grade.   The paper must be typed in 12pt Times New Roman or Arial with one inch borders and without grammatical error.

Every student will be emailed the template (outline) with the specific categories to be completed.

Use the Biopsychosocial Template for structuring your work.  List every Heading that is listed on the template and enter the appropriate information under this heading.  I will not hunt around your paper for the correct information.

Papers must be typed in 12pt Times New Roman or Arial with one inch borders and without grammatical error.

NEVER use a client's name, ever.  Confidentiality is vital.  No first names or last names.  Mr C, Miss T, Ms D or just X.  Do not make up a name.

Upload finished paper, at least 1500 - 1800 words, using a .doc, .docx or .pdf file.

The paper is worth 100 points and is worth 20% of your total grade.  Due 12/11/21
