LONG-TERM CARE FACILITY PROJECT INSTRUCTIONS

Part 1:

• Locate and contact a long-term care facility in your area. For this assignment, long-term care facilities include assisted living facilities, nursing homes, or retirement communities. See your textbook for specific definitions (ch. 14).

• Being mindful of the assignment due date in Module/Week 7, schedule a tour with a representative of the facility (administrator, director of nursing, admissions director, marketing, community or public relations representative, etc.), and obtain information according to the list of required slides provided below. Feel free to add other questions to your interview experience. Dress professionally and show gratitude to the long-term care facility representative for the time spent.

Part 2:

• After completing your tour and interview, design a PowerPoint presentation with a minimum of 10 slides that documents your observations, interview questions, and answers.

• The PowerPoint slide content must follow the order of the outline below.

o Creativity in design and appropriate use of pictures is encouraged.

o On the last slide, reference any sources used (including personal interview with the facility’s representative) in current APA format.

o PowerPoint slides must be in outline form. Any necessary narrative must be included in the notes section below the slide.

• The PowerPoint presentation will be submitted via the Module/Week 7 assignment submission link and in Discussion Board Forum 3 to be shared with your classmates.

Follow the outline provided below when creating slides for your PowerPoint presentation.

1. A title slide including your name, Liberty University, HLTH 380, Long-Term Care Facility Project.

2. List the name and location of the facility you toured as well as the date you toured the facility. How does this facility describe its operation (e.g., assisted living, retirement community, nursing home, etc.)? (1 slide)

3. What is the title of the person providing the tour and answering the interview questions? How does this person describe his or her job function at this facility? (1 slide)

4. What types of services are offered at this facility or by this facility (e.g., special units for dementia patients, rehabilitation services, counseling, etc.)? How is one admitted to this facility? Is a referral needed? (1 slide)

5. What level of nursing care is available and for what portion of the day (i.e., is a registered nurse available 24 hours on site)? What is the nurse-to-patient ratio? (1 slide)

6. What health professionals are on staff or on contract with this facility (e.g., physical therapists, recreational therapists, nurses, doctors, dietitians, social workers, etc.)? (1 slide)

7. What types of housing/shelter are available at this facility for residents (e.g., patient rooms, apartments, cottages, etc.)? Is transportation provided for residents to off-site locations such as doctor appointments or activities? (1 slide)

8. What types of recreational activities are available to the residents? Are residents who are unable to leave their rooms still able to participate in activities? Who plans the activities? (1 slide)

9. Does this facility have an active volunteer program? (1 slide)

10. Provide your overall impression of this facility based on your observation (note environment condition, staff to patient relationships, etc…). (1 slide)
