For this assignment, you are tasked with addressing a health disparity in your community.

Healthy People 2020 (Healthy People 2020, n.d.) defines a health disparity as a "particular type of health difference that is closely linked with social, economic, and/or environmental disadvantage. Health disparities adversely affect groups of people who have systematically experienced greater obstacles to health based on their racial or ethnic group; religion; socioeconomic status; gender; age; mental health; cognitive, sensory, or physical disability; sexual orientation or gender identity; geographic location; or other characteristics historically linked to discrimination or exclusion."

To complete this assignment, do the following:

1.            Identify a health disparity in your community and develop an intervention, policy, or program to address the disparity.

2.            Use the decision tree template from the Decision Trees lecture (download it here) or a different template that you create in Word, PowerPoint, Visio, or another flowcharting tool to show how you came up with the intervention/program/policy that will best meet the needs of your community.

3.            In 2-3 pages, assess which population with health disparities is present in your community and how the decision tree helped you develop the intervention, policy, or program for this population.

Some examples of groups that are susceptible to health disparities include, but are not limited to, the following:

•             Racial and ethnic groups

•             Lesbian, gay, bisexual, transgender, and questioning (LGBTQ) populations

•             People with disabilities

•             Transition-age youth

•             Young adults
