Health Promotion Program - Identification and description of Health Promotion Theory
The purpose of this assignment is to evaluate a health promotion program aimed at a particular population and for a certain health condition. The program chosen should focus on health promotion of adolescents, adults, or older adults. Do not choose a program with a primary focus on infants or children. Examples include rural health—"Health Coaches for Hypertension Control"; CV disease in women; stroke prevention; "Eat Smart Move More." If you have any questions about your chosen topic, please discuss it with Dr. Henson.
In your evaluation of the program, identify the following:
Underlying health promotion theory upon which the program is based
Target population: eligibility to participate in the program and social determinants of health affecting this population,
Strategies used to promote health and prevent disease consequences
The program cost and how is it funded
Efficacy: effect on health outcomes
Healthy People 2030 objective(s) address by this program
You will write a 5-7 page paper for your findings. APA formatting is required. The page limit does not include the title or reference pages. Remember to include an introduction and conclusion to your paper.
