In regards to family health risk, read the case study below and answer the questions.  The local health department was notified by the school counselor at the local high school of a 16 year old girl(student) who is  four months pregnant.  The student is in high school, lives at home with her mother, step father and younger sister. (Please reference Stanhope & Lancaster,  9th edition chapter 28 p.622-642, family health risks)

1.  What assessment tool would you use to collect more data about the family?

2.  Give example of two items you would ask to begin data collection.

3.  Briefly describe and provide your rationale for two things you would do to help this family prepare for the immediate future situation including the care of a newborn.
