Creating an addiction treatment plan is an individualized, dynamic process that begins at the first meeting with the client and evolves throughout the complete course of treatment in response to changing needs. An addiction treatment plan addresses the holistic needs of the individual. It includes the client’s immediate needs while in treatment as well as long-term needs once he or she returns to the community.

An addiction treatment plan is strengths-based. It identifies the individual’s inner strengths as well as the strengths found in the resources in the community. Some of the resources included in a strengths-based addiction treatment plan are those that address housing, vocational training, medical treatment, public assistance, mental health, and family counseling needs.

In this Assignment, you develop a 12-month addiction treatment plan for Marge using the "SPP Treatment Plan Template". This Assignment is part of your Scholar Practitioner Project.

For this Assignment,

For Week 5, you will develop a 12-month addiction treatment plan for   

               “Marge” using the Treatment Plan Template (found under Week 5

                Resources).

               Complete the Treatment Plan Template for “Marge”. Use the Treatment Plan Template

               to develop a hypothetical treatment plan for “Marge”. The plan should include

               “Marge’s” course of residential treatment, plans for discharge from residential

               treatment and aftercare plans for the next 12 months. It should identify 1.)

               Long-term goals, 2.) Short-term goals, 3.) Objectives, 4.) Strategies, and 5.)

               Expected outcomes (with timeframe).

               By Day 6, select a classmate’s posted treatment plan and provide feedback

               which identifies strengths and at least one suggestion for strengthening their

               treatment plan for “Marge”.
