Part 2: Identifying Social Determinants of Health (@ least 3 pages)

NOTE: Based on your community public health issue from Unit I, name at least four social determinants that must be considered when it comes to health program planning in your selected area. Be sure to justify your reasoning with evidence from scholarly literature.

For Part 2 of this project, due in this unit, you will address the following:

· Identify at least four (4) social determinants of health for your selected topic from Unit I that you must consider in health promotion planning.

· Discuss how you would go about planning for these determinants.

· Be sure to use scholarly journal articles and other peer-reviewed materials when discussing your determinants.

· Explain your rationale for choosing these determinants and why they should be considered.

Your completed paper should be at least three pages in length. You must use at least one outside source. All sources, including the textbook, must be cited and referenced according to APA Style. APA paper formatting (title page and running head) is not a requirement for this assignment.
