What do you usually do when you have a problem at work? Do you go straight to treating the symptoms of the problem, do you first look to determine if there is a deeper issue that needs your attention so the problem does not reoccur, or do you do something else? Think about the last major problem you had at work. Use Root Cause analysis to discover the actual underlying cause of the problem.
