Is the prevalence of health disparities leading to higher mortality rate in the United States (KFF, 2020).

Social and economic determinants contribute to health disparities in rural Areas; thus, leading to higher mortality rates (CDC, 2017).

4.Purpose Statement (1 page)

Identify and highlight how the utilization of Health Centers can address the social and economic determinants as it pertains to health disparities in rural areas (NCHI, 2017).

The research addresses the following research questions.

RQ1: What are the prominent health disparities in rural areas, and what are their causes?

RQ2: What implementations or steps can be taken to improve the social determinants of health, and health disparities in rural areas?

RQ3: Why is reducing the social and the economic determinants of health disparities in rural areas is important?
