Choose one of the following chronic diseases.  You will use the chosen chronic disease as the topic to address in each of the assignments in this course.

Chronic Obstructive Pulmonary Disease

Diabetes Mellitus Type 2

Childhood asthma

You must use a minimum of two scholarly sources.  This paper should be  4-5 pages in length.

In Assignment One you will:

Illustrate the population.  Who are the members of this population?  Provide details, including contributing causes for the disease and/or past medical history, any familial or genetic components for the disease.

Evaluate the population, including current numbers, the seriousness of the disease, special needs to optimize health and independence.

Assess the need for formal case management.  What are the current trends in case management (as discussed in your text) that support a case management model?

Argue the potential benefits to implementing a case management model, addressing economics, quality or life/care of the patient, impact on the social life of the patient and family.

Analyze why nurses should coordinate the case management plan of care?  Based upon best practice, what specialized training and experiences would a nurse bring to this role?

Identify the other members that should be included on a multidisciplinary case management team and discuss what their role is and why they are valuable members.

In this assignment, you will be exploring the use of evidence-based practice and how it is incorporated into case management patient care guidelines, and pathways based on your chosen chronic illness from assignment one.  You will identify and discuss current best practices, recommended published guidelines for treating or addressing the illness, and describe how clinical pathways help to address the importance of basing case management on current practice.

Provide support from a minimum of two scholarly sources.  Paper should be 4-5 pages in length.

You will create a fictitious patient based on your family assessment and use the attached Discharge Plan Template document to create a discharge plan for that patient.  Save the Discharge Plan Template to your files.  You can type right into the document, save it, and submit for grading.  You are to complete all sections of the discharge plan:  assessment, diagnosis/plan, education needs, financial worksheet, and the reflection and conclusion.  You need to be as detailed as possible in filling out all the boxes. The reflection and conclusion section allows you to summarize the patient’s plan of care based on all proceeding information and to make recommendations for the plan of care for the patient.  Reflect on what you learned in this assignment.  What do you think about using this discharge plan, did it help you to consider areas you might not have previously included in a discharge plan?  Please follow the Example Discharge Plan provided for your guidance.

Having learned more about case management through the readings in this course and first three assignments, for the final project you are creating a plan to implement case management at your workplace.  For the first section of the rubric, what is your recommended plan for the use of case managers in your organization for patients with your chosen chronic illness from assignment one?  In the second section, identify people within your organization who are stakeholders or would support your plan.  Whose support do you need to get your plan implemented?  In the third section, identify your goals – what do you hope to accomplish with your case management plan?

Often, starting with our goals helps - it’s a backwards design.  As we think about a case management plan we are proposing for our workplace, what do we want to accomplish?  Remember – goals should be measurable.  For example, if the chosen chronic disease is diabetes, one goal may be:

Clients will have decreased incidences of hyperglycemia requiring hospitalization

Once we determine what we want to accomplish, we can begin to construct our plan to achieve the goal.  To achieve this goal, our plan may include providing education in a manner the client can understand (being sensitive to cultural needs), access to phone support, or inputting glucose readings into their electronic health record for documentation the case manager can evaluate and use to reach out to the client.

To put this plan in action, whose support would we need?  Primary Care Physicians, Utilization Review personnel, and the Chief Financial Officer would all have an interest in optimizing patient health and reducing costs.

This paper should be 6-9 pages in length with a minimum of three scholarly sources for support.
